One question I have when reading the account was with this line at the beginning:

This had been happening for a while and, unfortunately, due to some external obstacles, internal staff were not available to investigate the issue–they had their hands full with an existing higher priority project.

What kind of “higher priority” prevents staff from dealing with a system that double-books orders? Taking orders correctly is a fundamental part of any kind of online retail system, and a flaw with that functionality would be a drop-everything-and-fix-this-right-now issue at the e-commerce companies I’ve worked for.

It’s been a few years, but if I recall correctly they were working on getting a new system up and running. The double orders were on the order of 10% and they could be dealt with manually.
